Description: Select Positions within a Malcolm Baldrige Award Winning Hospital!

Under general supervision, follows all established policies and procedures. Interviews patients and family members to obtain social history. Evaluates insurance coverage. Communicates with external payors. Completes and updates medical records pertaining to social work services. Coordinates and completes discharge plans. Counsels and educates patients and their families. Obtains follow-up information and resources. Gathers data and compiles reports on social work activities. Provides identified services to adult and geriatric populations.

Requirements include:

- Hours vary per position, but include Full and Part-Time as well as PRN and Per Diem
- Masters Degree required.
- LCSW preferred.
- Minimum of 1 year field experience.
- Requires strong interpersonal therapeutic and group therapy skills in order to counsel patients/families, interact with other hospital staff and external community agencies in a caring, courteous, professional manner.
- Requires the ability to assess patient and family needs and to develop and implement discharge plans.
- Support and fulfill the mission and values of SSM.
